In order to facilitate your initial appointment, please fill out the Health History Form and the Consent to Treatment Form and bring them with you. The New Patient Form may answer any additional questions that you may have about your appointment/treatment. Please feel free to contact us with any other concerns or questions at least 2 days before your appointment.
